Thank you for your interest in Disability Resources (DR) at Coconino Community College. DR provides individualized academic support for students who have a documented disability and qualify for accommodation services.
Below you will find links regarding required documentation based on specific disabilities, a FAQ sheet, Request for support, and Release of Information for both off-campus and on-campus entities. As a first step toward starting a file with DR, please complete the form and return to DR via the postal service or fax number at the bottom of this page. We will then contact you for an initial intake appointment with one of our DR specialists.
Please be aware the time required to review documentation and establish accommodations may take from 6-8 weeks. Therefore, submitting the necessary paperwork ahead of time will help to expedite the process and ensure you receive the services you need in order to be a successful student here at Coconino Community College.
